Therapy Care Management™ (TCM) has you covered!  TCM is a monthly in-person visit that combines our unique functional assessment (M-FRI™), vital sign screen, and a home safety review designed to identify issues before they become life changing problems.  Additionally, it is designed to recommend services, equipment, and safety needs.  This program adds a layer of support for doctors, families, and patients for those who have on-going conditions, such as osteoarthritis, COPD, Parkinson's Disease, ALS, Multiple Sclerosis, and more...

Vital Signs

A combination of blood pressure changes, heart rate reaction, and oxygenation can lead to symptoms that can cause a fall, or even fainting!

A tour of your home with recommendations for devices and tips for improving safety and accessibility.

Each of our assessments are followed by a routine check-in with your doctor!  The best team is a connected team!

Monthly In-Person Check-up

Monthly Check-Ups are scheduled in advanced with your physical or occupational therapist.  They come to you to help you identify any changes and work through any challenges!

Medicare Part B is a great insurance plan that provides for its beneficiaries; however, like most things, it has its limitations.

  • Each year your benefit for outpatient therapy may have a limit that may hinder your access to certain services, such as physical, occupational, and speech therapy.

  • Medicare Part B only covers a portion of your therapy services (80%).

  • Therapy services after a major life changing event can be needed for an extended time.

  • Some services may be denied after being rendered.


  • M-FRI: Our proprietary multi-factor fall assessment screening various systems that can lead to a higher incidence of falls.

  • Monthly In-Person Follow-up Assessments

  • Primary Care Physician Communication

  • Consistent Therapist, with options of Physical, Occupational, or Speech Therapist

  • In-Home Assessments

At anytime, if additional services are needed, such as physician care, nursing, physical, occupational, or speech therapy, social work, or other services, we will work with your doctor to help coordinate care.
